Transitions in Care (TiC) Fall Stregthening Workshop Summary

On behalf of the CIHR Transitions in Care (TiC) scientific leadership, it is our great pleasure to share with you the Workshop Summary from the CIHR TiC Initiative Strengthening Workshop.

The CIHR TiC Initiative is a multi-Institute, trans-disciplinary, CIHR priority-driven health research initiative, aimed at improving transition in care outcomes for all Canadians. Poor outcomes from inefficient and ineffective transitions in care have been longstanding concerns within healthcare organizations and systems and identifying solutions is a priority for patients/families, clinicians and those who manage our health system.

On September 7, 2017 more than 60 thought-leaders in the field attended a one-day workshop in collaboration with the CIHR host Institutes. The workshop objectives included:

  • Identify actionable research gaps and opportunities to advance the collective and measureable impact of the CIHR TiC Initiative;
  • Identify where Canada can add value to advance research on care transitions;
  • Identify opportunities for advancement of transition in care research across the CIHR research themes;
  • Identify, develop and engage partners in the development of the initiative.

The Workshop Summary provides a detailed overview of the event’s discussion and includes an overview of the key themes as well as crosscutting themes identified by those in attendance.

The CIHR TiC Initiative scientific leadership would like to thank all those who participated in the strengthening workshop for their time, input, advice and comments throughout the day; these provided us with important insights for the development of this exciting new CIHR multi-Institute initiative.
